IV sedation, atraumatic extractions, and six precisely angled implants placed in a single 4-hour visit. Your same-day arch is screwed in before you go home.

Integration (3–4 months)
Bone fuses to titanium. The Vampire Protocol shortens this window by 30%. You eat soft food for 2 weeks, then progress to a normal diet. Dr. Qiu checks integration at 2, 6, and 12 weeks.

Case A: Full-arch reconstruction
R.V., a 58-year-old woman from Whittier, arrived at 5D Smiles with a failing upper dentition: eleven remaining upper teeth, eight of them with periapical pathology, three with vertical fractures. She had been advised by two other offices to remove all remaining upper teeth and wait six months before beginning implants. Dr. Qiu reviewed her CBCT and determined that same-day arch placement was feasible at four posterior anchor sites, with the two anterior implants placed into immediate extraction sockets. A targeted sinus lift was needed at the upper right second molar position and was performed the same day.
Eleven extractions, six implant placements, bilateral PRP bone graft, and a same-day provisional arch were completed in a single five-hour surgical visit under IV sedation. R.V. left the office with fixed teeth. She progressed through the integration period on a soft diet, with check-ins at two, six, and twelve weeks. Integration was confirmed at 16 weeks, and the final monolithic zirconia arch was delivered at week 18. At her 12-month follow-up, bone levels at all six implants were within 0.5 mm of the immediate post-surgical baseline, which is consistent with the Vampire Protocol outcomes in the published UCLA data.

Case B: Smoker revision after failed All-on-4
J.L., a 64-year-old male, came to us 22 months after receiving an All-on-4 restoration at a different practice. He had smoked one pack per day for 38 years and had quit at the time of his original surgery but resumed within six months. At presentation, two of his four implants had failed due to peri-implantitis, and the arch was mobile. The surviving two implants were osseointegrated but showed 3 mm of crestal bone loss on CBCT. He had been told by his original office that nothing could be done without a full rebuild, and that smoking made him a poor candidate for further implants.
Dr. Qiu proposed a revision plan: remove the failed implants and the compromised arch, treat the peri-implant infection, place four new implants at revised positions using the UCLA Vampire UV-photofunctionalization protocol (which improves osseointegration in high-risk patients by modifying the titanium oxide surface), and retain the two surviving implants as additional support after the bone loss was addressed. A six-week smoking cessation protocol was required before surgery, and J.L. complied. The Vampire PRP graft was used at both failed implant sites to regenerate lost bone.
All six implants integrated. Final zirconia arch delivered at week 20. At 18-month follow-up, J.L. remains a non-smoker, bone levels are stable, and he describes full chewing function. He is the patient referred to in our FAQ as an example of a failed All-on-4 converted to All-on-6. Total revision cost: $14,500 (reduced from full-arch price because two original implants were retained).
