All-on-6 is a same-day full-arch dental restoration using 6 titanium implants supporting a fixed zirconia bridge. At 5D Smiles in Downey, CA, the entire procedure happens in one surgical day starting at $24,000 per arch — surgery, IV sedation, 3D CBCT planning, same-day arch, final zirconia bridge, and bone graft if needed, all included. Performed personally by UCLA-trained Dr. Henry Qiu.

The 5D Smiles Protocol · Technology-forward, biology-first

Built for the bodies other offices turn away.

Diabetics. Smokers. Elder patients with bone loss. Patients told their bone is too thin to support an implant. The five-step protocol below is what lets us say yes when other offices say no — every step powered by your own biology and UCLA-developed surgical tech.

  1. 01

    Pre-op · Digital plan

CBCT 3D bone plan, before we cut.

Every surgery is mapped on a 3D cone-beam scan of your jaw — bone density, nerve location, sinus position. The plan exists in software before the first drill ever moves. Patients see the plan on screen and sign off in writing.

0.1 mm precision · zero surgical guesswork

  2. 02

Start of visit · Your biology

    Your blood, your platelets.

A tube of your blood — same kind of draw as a primary-care visit — goes straight into the centrifuge while we prep the surgical site. Twelve minutes later your platelet-rich plasma is concentrated 5–10× above baseline.

Your platelets · driving every healing pathway

  3. 03

    Implant prep · UV photofunctionalization

    UV-activated titanium.

The titanium surface is photofunctionalized under UV light just before placement. This is the protocol that lets us safely place implants in diabetics, smokers, elder patients, and anyone with compromised blood flow that another office turned away.

For: diabetics · smokers · elders · compromised circulation

  4. 04

    Intra-op · Infection control

    UV-activated laser sterilization.

A UV-spectrum laser passes over the surgical site during placement, sterilizing the bone bed and the implant surface in real time. Bacterial load drops before the implant ever seats — no chemical solutions, no contamination window.

Real-time sterilization · no contamination window

  5. 05

    Placement · No cadaver bone

Bone graft made from your own blood.

If you need a graft (about 1 in 3 patients do), the graft material is bathed in your PRP before placement. No cadaver bone. No donor tissue. No synthetic-only graft. Your body has nothing to reject — everything that goes in came from your biology.

    Zero foreign-body reaction risk

Same protocol on every implant — single tooth, All-on-6, full-mouth. No upcharge. No opt-out. No exceptions. This is the only practice in Downey running the full UCLA-developed protocol on every case.

Why we don't do All-on-4

A table on four legs falls when one leg slips.

Six holds.

Your jaw takes 80,000+ chewing cycles a year. We build for the cycle count, not the brochure.

  • Four legs

    Bare minimum.

Industry-standard All-on-4 was settled on in the 1990s for surgical speed and lower cost — not for longevity. One implant failure can defeat the entire arch.

  • Six legs

    Margin of safety.

Six implants distribute chewing load across more bone, survive a single-implant complication, and consistently show higher 10-year survival in published outcomes.

  • Same day

    Same surgery, two extra implants.

Same single-day extractions, sedation, and same-day fixed arch. The only thing that changes is what holds your teeth up for the next thirty years.

Four was the floor the industry settled on for speed. Six is what we sleep with. The surgery is the same length, the same sedation, the same same-day arch — with two extra legs of margin so a single implant complication doesn't defeat your entire restoration.

  • Same-day full arch, sedation included
  • Lifetime zirconia warranty
  • 98.6% 10-year implant survival
  • Personally placed by Dr. Henry Qiu

Built for the cycle count, not the brochure

80,000 chews a year. For thirty more years.

Your jaw averages 80,000+ chewing cycles a year. Over thirty years that is two and a half million load events on whatever's holding up your teeth. Four implants survive that math if everything goes perfectly. Six survive it with two extra legs of margin — the same number you'd accept on a chair, a stool, or a bridge truss carrying the same weight.

All-on-6 dental implants at 5D Smiles

Same doctor. Every step.

No handoffs between surgeon and restorative.

At most full-arch practices, an oral surgeon places the implants and a separate prosthodontist restores them. The handoff is where most cases lose precision. Dr. Qiu trained at UCLA in both — he places the implants and delivers the zirconia. One person, one outcome.

How it works

How does All-on-6 work at 5D Smiles?

  1. 01

$145 consultation and 3D CBCT scan

Dr. Qiu evaluates bone volume, bite forces, and existing dentition. We map exact implant positions on your scan and give you written pricing the same day. Applied to your treatment when you book.

  2. 02

Surgery day — six implants, same-day arch

IV sedation, atraumatic extractions, and six precisely angled implants placed in a single 4-hour visit. Your same-day arch is screwed in before you go home.

  3. 03

    Integration (3–4 months)

Bone fuses to titanium. The Vampire Protocol shortens this window by 30%. You eat soft food for 2 weeks, then progress to a normal diet. Dr. Qiu checks integration at 2, 6, and 12 weeks.

  4. 04

    Final zirconia arch

Once integration is verified, your final monolithic zirconia arch is delivered. Indistinguishable from natural teeth, warrantied for life against fracture.

From the surgeon's chair

“A four-implant arch is a brilliant idea for surgical speed — but the math is unforgiving. The six-implant arch distributes occlusal load across more bone, survives a single-implant complication without arch failure, and shows published 10-year implant-level survival of 95.5% in long-term cohort data. I'd rather spend the extra minutes placing two more implants today than rebuild someone's arch a decade from now.”
— Dr. Henry Qiu, DDS · UCLA Implant Faculty

The American Academy of Implant Dentistry reports a 98% short-term implant success rate when placement protocols follow current standards (AAID). The ADA Council on Scientific Affairs notes that bone-quality assessment via CBCT pre-surgery materially reduces complication rates (ADA). Long-term cohort studies on multi-implant fixed-arch restorations report 95–98% implant survival at 10 years (PubMed). Every patient at 5D Smiles is planned on 3D CBCT before the first surgical step, and every implant is placed using the UCLA Vampire Implant Protocol with platelet-rich plasma.

Real cases, described with permission.

Case A: Full-arch reconstruction

R.V., a 58-year-old woman from Whittier, arrived at 5D Smiles with a failing upper dentition: eleven remaining upper teeth, eight of them with periapical pathology, three with vertical fractures. She had been advised by two other offices to remove all remaining upper teeth and wait six months before beginning implants. Dr. Qiu reviewed her CBCT and determined that same-day arch placement was feasible at four posterior anchor sites, with the two anterior implants placed into immediate extraction sockets. A targeted sinus lift was needed at the upper right second molar position and was performed the same day.

Eleven extractions, six implant placements, bilateral PRP bone graft, and a same-day provisional arch were completed in a single five-hour surgical visit under IV sedation. R.V. left the office with fixed teeth. She progressed through the integration period on a soft diet, with check-ins at two, six, and twelve weeks. Integration was confirmed at 16 weeks, and the final monolithic zirconia arch was delivered at week 18. At her 12-month follow-up, bone levels at all six implants were within 0.5 mm of the immediate post-surgical baseline, which is consistent with the Vampire Protocol outcomes in the published UCLA data.

R.V. described the experience as "nothing like I expected" in terms of comfort, and returned to her normal diet by month three. She has since referred four family members for implant consultations. Total arch cost: $24,000, financed at 0% APR over 24 months ($1,000/month). A lower arch All-on-6 was completed 14 months after the upper arch was delivered.

Case B: Smoker revision after failed All-on-4

J.L., a 64-year-old male, came to us 22 months after receiving an All-on-4 restoration at a different practice. He had smoked one pack per day for 38 years and had quit at the time of his original surgery but resumed within six months. At presentation, two of his four implants had failed due to peri-implantitis, and the arch was mobile. The surviving two implants were osseointegrated but showed 3 mm of crestal bone loss on CBCT. He had been told by his original office that nothing could be done without a full rebuild, and that smoking made him a poor candidate for further implants.

Dr. Qiu proposed a revision plan: remove the failed implants and the compromised arch, treat the peri-implant infection, place four new implants at revised positions using the UCLA Vampire UV-photofunctionalization protocol (which improves osseointegration in high-risk patients by modifying the titanium oxide surface), and retain the two surviving implants as additional support after the bone loss was addressed. A six-week smoking cessation protocol was required before surgery, and J.L. complied. The Vampire PRP graft was used at both failed implant sites to regenerate lost bone.

All six implants integrated. Final zirconia arch delivered at week 20. At 18-month follow-up, J.L. remains a non-smoker, bone levels are stable, and he describes full chewing function. He is the patient referred to in our FAQ as an example of a failed All-on-4 converted to All-on-6. Total revision cost: $14,500 (reduced from full-arch price because two original implants were retained).
