Six implants — not four
All-on-6 uses six precisely angled titanium implants per arch. We never do All-on-4. The table analogy is clear — a four-legged table tips when one leg fails; a six-legged table doesn't. Two extra implants is the difference between a 97 percent ten-year survival rate and a single-point-of-failure case that loses the entire arch when one implant struggles.
Same-day fixed arch
You walk in toothless or with failing teeth and walk out with screwed-in teeth the same day. No removable denture phase. No going home with a flipper. The same-day arch is your provisional restoration until the final monolithic zirconia is delivered 3 to 4 months later.
IV sedation — included
IV sedation typically runs $750 to $1,500 in our market when billed separately. For All-on-6 cases at 5D Smiles, it's included in the $20,000. You sleep through the 4-hour surgical visit, wake up with same-day teeth, and go home the same day.
Bone grafting — included
About 1 in 3 All-on-6 cases needs targeted posterior bone grafting. Six implants need more posterior bone than four. The graft material is your own PRP-enriched autograft — no cadaver bone, no donor tissue. Other Downey offices charge $800 to $2,500 for this step. We include it.

Every All-on-6 case at 5D Smiles uses the Vampire Implants™ Protocol: UV-activated titanium surfaces, your own platelet-rich plasma at the surgical site, atraumatic extraction technique. Integration time is shortened by about 30 percent compared to standard-protocol implants — and survival at 10 years runs 97 to 98 percent.
10-year biological warranty
10-year biological warranty covers both the implant integration and the zirconia arch — surgery, parts, and lab included. Most offices warranty implants 1 to 5 years and exclude the prosthesis entirely. Our walk-out price is our warranty price.

Before you leave the consult, you walk out with a written single-arch and dual-arch quote: implants, sedation, same-day arch, final zirconia, warranty. Per-arch $20,000. Dual-arch $40,000 — one surgical visit, one sedation, one zirconia lab production run.

Insurance benefits verification
Most PPO plans contribute $1,500 to $3,000 toward an All-on-6 case. Our front desk verifies your benefits live before you leave. You see your real out-of-pocket in writing — not a marketing headline that drops the insurance contribution in the asterisk.

0% APR financing up to $60,000 (covers a dual-arch case in most situations). A $20,000 single arch is about $358 per month at 60 months once a typical PPO contribution is applied; a $40,000 dual arch is about $667 per month at 60 months, and a PPO contribution of $1,500 to $3,000 brings it to roughly $617 to $642. Subprime tiers approve down to ~580 FICO.

Surgery day — 4 hours, same-day teeth
IV sedation, atraumatic extractions, six precisely angled implants, and your same-day arch screwed in before you go home. The entire surgical visit is typically 4 hours per arch (or 5 to 6 hours for dual arch in a single visit). You drive home with teeth, not a denture.

Final monolithic zirconia arch
After 3 to 4 months of integration, your final monolithic zirconia arch is delivered. The Vampire Protocol shortens that window by about 30 percent. Designed to match your natural teeth, covered under our 10-year biological warranty, hand-finished by our local Downey lab.

All-on-4 saves about $3,000 upfront — and costs $15,000 to $30,000 in revision when a single implant complication takes out the arch. What I’m actually charging for
Here is the part most price-shoppers miss. The UV-activated implants I use cost about $400 each before a single other line item — the surgery, the sedation, the bone work, the titanium bar under the bridge, the final teeth. The most misleading ad I have ever seen was $10,000 an arch, and it is simply impossible to do properly at that number; to hit it an office uses roughly $100 implants and skips the bar and the aesthetics. The longevity evidence backs spending for quality: a 2023 systematic review of full-arch fixed implant prostheses (PMC) reports 97 to 98% implant survival at 10 years for six-implant restorations, and the American Dental Association cites a 90 to 95% implant success rate over 10 years when placement protocols are followed. The real cost is whether the arch lasts: an arch that fails sends you into a revision that, in my experience, runs about 50% more than doing it right the first time — the math I work through in my full-arch cost analysis.
