The most common surprise on a competing All-on-6 quote isn't the surgery — it's the line items added afterward. We list them up-front so you can compare apples to apples.
Six implants — not four
All-on-6 uses six precisely angled titanium implants per arch. We never do All-on-4. The table analogy is clear — a four-legged table tips when one leg fails; a six-legged table doesn't. Two extra implants is the difference between a 97 percent ten-year survival rate and a single-point-of-failure case that loses the entire arch when one implant struggles.
Same-day fixed arch
You walk in toothless or with failing teeth and walk out with screwed-in teeth the same day. No removable denture phase. No going home with a flipper. The same-day arch is your provisional restoration until the final monolithic zirconia is delivered 3 to 4 months later.
IV sedation — included
IV sedation typically runs $750 to $1,500 in our market when billed separately. For All-on-6 cases at 5D Smiles, it's included in the $20,000. You sleep through the 4-hour surgical visit, wake up with same-day teeth, and go home the same day.
Bone grafting — included
About 1 in 3 All-on-6 cases needs targeted posterior bone grafting. Six implants need more posterior bone than four. The graft material is your own PRP-enriched autograft — no cadaver bone, no donor tissue. Other Downey offices charge $800 to $2,500 for this step. We include it.
UCLA Vampire Protocol
Every All-on-6 case at 5D Smiles uses the UCLA Vampire Implant Protocol: UV-activated titanium surfaces, your own platelet-rich plasma at the surgical site, atraumatic extraction technique. Integration time is shortened by about 30 percent compared to standard-protocol implants — and survival at 10 years runs 97 to 98 percent.
Lifetime zirconia warranty
Implant body: 3 years against failure. Final monolithic zirconia arch: lifetime against fracture or wear. Most offices warranty implants 1 to 5 years and exclude the prosthesis entirely. Our walk-out price is also our warranty price.

Dr. Qiu personally evaluates your bone volume, bite forces, and existing dentition. We map exact implant positions on your 3D CBCT scan. The $145 is fully refundable when you book — it's a deposit, not a fee. Consult takes 60 to 75 minutes.

Written, itemized All-on-6 quote
Before you leave the consult, you walk out with a written single-arch and dual-arch quote: implants, sedation, same-day arch, final zirconia, warranty. Per-arch $20,000. Dual-arch $38,000 (a $2,000 savings vs. two separate arches because we use one surgical visit, one sedation, and one zirconia lab production run).

Insurance benefits verification
Most PPO plans contribute $1,500 to $3,000 toward an All-on-6 case. Our front desk verifies your benefits live before you leave. You see your real out-of-pocket in writing — not a marketing headline that drops the insurance contribution in the asterisk.

0% APR financing up to $60,000 (covers a dual-arch case in most situations). A $20,000 single arch with $2,500 PPO contribution becomes $17,500 financed — about $292 per month at 60 months. A $38,000 dual arch becomes about $585 per month at 60 months. Subprime tiers approve down to ~580 FICO.

Surgery day — 4 hours, same-day teeth
IV sedation, atraumatic extractions, six precisely angled implants, and your same-day arch screwed in before you go home. The entire surgical visit is typically 4 hours per arch (or 5 to 6 hours for dual arch in a single visit). You drive home with teeth, not a denture.

Final monolithic zirconia arch
After 3 to 4 months of integration, your final monolithic zirconia arch is delivered. The UCLA Vampire Protocol shortens that window by about 30 percent. Indistinguishable from natural teeth, lifetime warranty against fracture or wear, hand-finished by our local Downey lab.

All-on-4 saves about $3,000 upfront — and costs $15,000 to $30,000 in revision when a single implant complication takes out the arch. We use six implants on the same single-day surgery (same extractions, same sedation, same same-day fixed arch). The procedure is 30 minutes longer. The longevity difference compounds over decades.
