Insurance Plans We Work With
5D Smiles accepts most PPO dental insurance plans. We verify your benefits before treatment so you know exactly what you’ll owe.
PPO Dental Insurance
Most PPO plans offer 50% coverage on major procedures including implant restorations. We accept Delta Dental, Cigna, Aetna, MetLife, Guardian, and hundreds more.
HMO / DHMO Plans
HMO plans often have more restrictions on implants and may require referrals or limit coverage to certain providers. Call us — we’ll review your specific plan and find all available benefits.
Medical Insurance
Tooth loss caused by accident, injury, or medical necessity may be covered by medical insurance (not just dental). We submit to both dental and medical insurers to maximize your reimbursement.
Medicare / Medicaid
Traditional Medicare does not cover routine dental. Some Medicare Advantage plans include dental implant coverage. Denti-Cal (Medi-Cal dental) has limited implant coverage for qualifying conditions.
HSA / FSA Funds
Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) can be used for dental implants — they’re considered a qualified medical expense. Use pre-tax dollars to lower your out-of-pocket costs.
Veterans Benefits
VA dental benefits may cover implants for eligible veterans with service-connected conditions. Contact your VA dental coordinator to determine your eligibility before your consultation.
What If Insurance Doesn’t Cover Everything?
We offer multiple financing options to cover whatever insurance doesn’t — so cost never prevents you from getting the smile you deserve.
CareCredit
12–60 month financing for healthcare costs. Many plans offer 0% APR promotional periods. Apply online in minutes.
Sunbit
Flexible payment plans with high approval rates — even for patients with less-than-perfect credit. 85%+ approval rate.
In-House Payment Plans
We offer direct payment plans for qualifying patients — spread your treatment cost over time with no third-party lender required.
Split Billing by Year
If you’re starting a multi-stage treatment, we can schedule procedures across calendar years to maximize your annual insurance benefits.
Dental Implant Insurance FAQ
Does dental insurance cover dental implants?
Most PPO plans cover part of implant treatment — typically 50% of the restoration (crown) and sometimes the abutment. The implant post itself is often not covered. Your annual maximum (usually $1,000–$2,500) limits total reimbursement.
How do I know what my insurance covers?
5D Smiles verifies your insurance benefits for free before your consultation. We contact your insurer directly, get a detailed breakdown of covered procedures, and provide a clear cost estimate before any treatment begins.
What is the “missing tooth clause”?
Many insurance plans exclude coverage for teeth that were missing before your coverage began. This is called the “missing tooth clause.” It’s a common gotcha — we check for this in your benefits verification.
Can I use my FSA or HSA for dental implants?
Yes. Dental implants qualify as a medical expense under IRS guidelines. Using FSA or HSA funds lets you pay with pre-tax dollars, saving 20–35% depending on your tax bracket.
What if I don’t have dental insurance?
No insurance? No problem. We offer in-house payment plans, CareCredit, and Sunbit financing. Many patients without insurance find our monthly payment options more affordable than expected — and our free consultation includes a full cost breakdown.
